Refer a Patient

Because of our established reputation in the field of neurosurgery, you should feel confident entrusting us with the care of your patients. By combining your input, our neurosurgical expertise, and the latest technological advancements, your patients will receive an unsurpassed level of care.

Please give the following information to the patients you are referring to our practice:

• They will receive a reminder call about their appointment
• They will receive a registration information packet in the mail for them to complete. They must bring this with them to their appointment.
  They need to bring copies of all X-rays, MRIs, or scans from any facilities not affiliated with CaroMont Imaging or Gaston Memorial Hospital. If scans were performed at GMH, the Diagnostic Center, CaroMont Imaging – Summit or CaroMont Imaging – Belmont, they do not need to get copies of these. Our offices can access these through the PACS system. Patients can call our office at 704-864-5550 with questions if they are not sure about getting copies.
• They must bring ALL current medications.
• They must bring their insurance card and co-pay.
• New patients must arrive 30 minutes before appointment time. Follow-up patients must arrive 15 minutes early.
• We require 24-hour notice if they are unable to keep an appointment.
