Welcome to our practice. We want to make your visit as comfortable as possible, so please take time to read this information and the attached forms carefully to ensure all paperwork is filled out correctly.
 
You will need to bring the following information with you to your appointment:
Completed attached new patient forms. If it's not possible to complete these prior to your appointment, please arrive about 30 minutes early to fill out this information.
Medical insurance cards and authorization paperwork.
Any medical records, test results, X-rays, CT Scans, and MRIs from other physicians who have treated you for your current condition.
A list of all medications you are currently taking.
 
These items are very important and enable our physicians to provide the proper care and treatment you need.
 
All co-pays and co-insurance amounts will be collected at check-in. If the reason for your visit is due to an accident or work-related injury, please contact our office immediately so we can determine payment responsibility.
 
If you are unable to make your appointment, a 24-hour cancellation notice is required or you may be billed for the appointment.
